The first two weeks after surgery are when most preventable readmissions happen. A senior who lives alone, on pain meds, with mobility restrictions and a surgical incision is one stumble away from going back.
We step in for those critical weeks. Sometimes it's 24/7 right after discharge, scaling back to daytime-only as recovery progresses. The goal is simple: get them through the recovery window safely so they can finish recovering on their own.
