The care journey begins with an initial assessment. Our experienced Wellness Liaison will meet with you and your loved one to discuss their specific needs, preferences, and goals. We will gather comprehensive information to create a personalized care plan that addresses your unique expectations, needs, and wishes. To do this we will need any specific physician, physical therapist, and family directions, including wants and wishes of each individual senior that we assess.
Custom Care Plan Development
Based on your assessment, our team will develop a customized care plan tailored to your loved one’s individual needs. The care plan outlines the specific services and support they will receive as requested and found in the Initial Assessment, including personal care, medication support, companionship, meal preparation, and any additional assistance, tasks, or chores required. Basically, you tell us what you want, and we can usually do it.
Introduction and Orientation
Once a suitable caregiver is selected, we will introduce them to your loved one. This introductory meeting allows them to get to know each other, establish rapport, and discuss the care plan in detail. The caregiver will familiarize themselves with your loved one’s routine, preferences, and any specific instructions. Please allow a bit of time for this process. If at any time it’s not a match, just let us know!
Regular Communication
We believe in maintaining open lines of communication with our clients and their families. Our Wellness Liaison and caregiver will regularly update you on your loved one’s progress, any changes in their condition or needs, and address any concerns or questions you may have. We value your input and strive to provide the highest quality communication of care progress!
Ongoing Monitoring and Adjustments
Our team conducts regular evaluations to monitor the effectiveness of the care plan and make any necessary adjustments. Therefore, we proactively assess your loved one’s changing needs, adapting the care plan as required to ensure they continue to receive the best possible care and support.
Family Involvement
We encourage family involvement and value the input and feedback from our clients’ and their families. We welcome your participation in care planning meetings, regular visits, and ongoing communication. Together, we can provide the highest levels of care and create a supportive and nurturing environment for your loved one. When families cannot be involved, we can give families extra comfort and help through kind and loving relationships by building lasting relationships.
